The song was first shown in Gilmour's live performances in 2001 and 2002 (as well as on the accompanying DVDs) where Gilmour introduced it as "This is a new one so if you're bootlegging, start your machine now. " recorded at the houseboat studio, Astoria, for the 2006 album On the Island . The song is within 3/4.

For three nights at the Royal Albert Hall during the On an Island Tour in 2006, blank CDs with single cover pads Smiles, and CD-stickers left in the audience seats that will find them on arrival, each carrying a unique number. The idea is to download a single from iTunes, burn it to a CD-R, put a CD-shaped sticker onto a burned CD and save it in the wallet.

"Island Hour" was originally available via the Gilmour website before it was made wider available.

An unhealthy "smile" form can be heard briefly on the BBC2 Three Men in a Boat event that retraces the journey on the River Thames, and visits the houseboat.
