Plot

Seok-woo, a divorced fund manager, is a workaholic dad and absent for his young daughter Su-an. For his birthday the next day, he hopes his father takes him to Busan to see his mother. They boarded the KTX at Seoul Station. Others on the same train were Sang-hwa's hard-working husband and his pregnant wife Seong-kyeong, a high school baseball team, a self-centered, self-sufficiency COO Yon-suk, In-gil and Jon-Gil, and a homeless people with post-traumatic stress disorder.

As the train departs, a struggling young woman rides the train with a bite wound on her leg. The woman immediately became a zombie and attacked a flight attendant, who later also turned into a zombie. The infection quickly spread throughout the train. Yong-guk baseball player, a girl named Jin-hee who has a crush on him, and several passengers managed to escape to another car. News releases report zombie outbreaks across the country. The train stopped at Daejeon, but surviving passengers found that the city had also been ridden, and they hurriedly retreated back to the train, splitting into different train cars in chaos. The conductor revives the train to get to Busan, where a successful quarantine zone is reported to have been established.

Seok-woo, Sang-hwa, and Yong-guk fight to where Su-an, In-gil, and Seong-kyeong are hiding, and together they fight through a horde of zombies into the front wagon, where other passengers are sheltered. However, on Yon-suk's initiative, the passengers blocked the survivors from entering, afraid that they were infected. Sang-hwa and In-gil sacrifice themselves to give others time to force open the door and enter the car. Yon-suk demands that newcomers isolate themselves in the vestibule, and others follow in his footsteps. When Jong-gil accidentally opens the door to a zombie to be with his zombie sister, In-gil, the zombies kill the rest of the passengers, leaving Seok-woo, Su-an and Seong-kyeong safe, as they are in the front room. Yon-suk and train officers escape from the onslaught by hiding in the bathroom.

The blocked lane at East Daegu train station forced the victims to stop and look for another train. In the process, Seok-woo, Seong-kyeong, Su-an, and homeless man are separated from Yong-guk and Jin-hee. Yon-suk escapes after pushing the train officer to be killed by the zombies, then does the same with Jin-hee. Heartbroken, Yong-Guk stays with Jin-hee and gets bitten by him. The train conductor starts the locomotive on another track but is also killed by a zombie while trying to save Yon-suk. The homeless man sacrificed himself to let Su-an and Seong-kyeong run away with Seok-woo into the train that the conductor had activated. They meet Yon-suk inside the helicopter cabin, on the verge of turning into a zombie, bitten as the train conductor rescues him. Seok-woo fights him, but he himself gets bitten. He places Su-an and Seong-kyeong inside the engine room and shares his final words with his daughter before moving out. When he zombifies, he thinks about the first time he holds his daughter in his arms and throws himself from the locomotive with a smile.

Su-an and Seong-kyeong get off in Busan and start walking through the railway tunnel. On the other side of the tunnel are soldiers stationed to defend the perimeter against zombies. Unable to see the new arrival clearly, the soldiers at the checkpoint were ordered to shoot them. However, the soldiers then heard the singing, which made them realize that the newcomer is human: Su-an, crying while singing the song she wanted to do for her father at the beginning of the movie, now sings it in honor of her sacrifice..
